Thesis Intent Form

Students interested in completing the Science Research Distinction Certificate should complete and submit a thesis intent form when a research mentor is identified. Please check with your department on specific requirements for thesis submission.

Part 1: To be completed by student
This is your 9-digit PSU ID number
Please check with your department to find out who needs to sign and approve your thesis -- the department head or the director of undergraduate studies.
Enter the semester and year of the semester you plan to graduate. For example, FA14 or SP15

Part II: Student and Adviser Agreement

Students must engage in research activities over the course of at least 3 semesters or 2 semesters and a summer. Regular meetings between the student and research mentor should occur over the course of the 3 semesters or 2 semesters and a summer. The student will be enrolled in and complete 6 credits of independent research in the Eberly College of Science. During the semester the thesis is submitted, the student will be enrolled in SC 494 section 002 (1 credit).  Please check with your department on specific requirements for thesis submission.

As the student’s research mentor, you agree to the mentor him/her throughout the project and will be available for regular meetings and draft/performance reviews. Your signature below indicates that you agree to the responsibilities of a research mentor and that you approve of this student’s thesis proposal.
